a theory that I haven't heard yet is that Price is mentally losing it, after the gas exposure, we never got anything more than him passing out, when Soap "died" it was in Price perspective, when the outro cutscene happened, it was in Price perspective, Soap could've easily been someone else, possibly Roach or someone else, and Price could be seeing Soap instead
