In what way did he become a burden on your health care system? He'd still pay for his own bills and medical care, no? I don't think it's any more of a point than someone choosing to smoke or engage in anything that causes harm to their overall health. Assuming he actually had his health failing him aside from getting obese. Any obese person can put on an oxygen tank when they don't actually need it.

If everyone in a society lived a horrible lifestyle and thus died prematurely, it follows that such a society had paid a horrible price for its collective choices (assuming it knew beforehand that its choices were unhealthy).

However, such a society might insist it made a good choice -- that it enjoyed its short lifespan more than a longer -- but less satisfying -- lifespan/lifestyle.

But maybe because he did that and many people saw that and become more aware not do it or maybe some other people thought that this looks unhealthy and destructive that they won’t eat fast foods altogether (and that in return put less pressure on healthcare system ). Many comments under his videos was something like: this is motivating me to go to gym.
