Chvostek's sign (Semiopedia: Pathophysiology of semiotics) by Dr.Harinath, DMA, Chennai

Chvostek's sign:
It is characteristic of hypocalcemia. Hypocalcemia lowers threshold Of opening of the Na+ channels resulting in increased neuromuscular excitability.
Chvostek's sign and Trousseau's sign are obviously the presentations of increased neuromuscular excitability that can be seen even in subtle cases of hypocalcemia.
Chvostek's sign: it is the ipsilateral contraction of facial muscles elicited by the tapping of the facial nerve just in front of the auricle as you see in this video
Trousseau's sign: It is the carpal spasm after the inflation of the cuff of a sphygmomanometer, 10 mmHg above the systolic pressure for 3 minutes.
DDx of Chvostek's sign:
1. Hypocalcemia of any cause: Hypoparathyroidism, ↓vitamin D, Pseudohypoparathyroidism, Pancreatitis & Hyperventilation/respiratory alkalosis
2.Less commonly: Hypomagnesaemia
