I wrote support about it and they said, that a secondary officer leads as well... But my observations were just as yours.

about spanner, i do believe that even if he's placed as a first officer (not leader/ captain) he still provides that damage resist buff, you can check with high effect quality, that the yellow shield effect beneath them troops, are still applied being deployed as first officer (not leader/ captain)
same case with antonina mentioned below by trang bang, it was a test made by HunterBBQ, that Bald (Eruptor guy) + Antonina deals more damage than Antonina + Bald to enemy bases, and War Machine + Antonina deals more damage than Antonina + War Machine to enemy troops (and i also tested it, and it's correct, assuming every officers who joins in the test is level 60, maxed out)
so what i'm trying to say here in this game, is that "when leading" means, you only need to use a correct troop type.
For ToTS especially, there was a description said in the patch note that: "When he's being deployed as a captain (leader), he reduces the prep time of officer skill charge by 1.5 secs", it's correct in this case, but somehow there is no such description in the game, which leads to a misunderstanding.

So after I read through the comments: Basically it doesnt matter which officer you put first or second, the only exception is Tip of the Spear. He has to be first. Am I correct? 🤔

Other than not being max 60, is there any reason why you keep Percy as second officer? Wouldn’t she be better as primary? as her skill should boost who ever you have as her secondary officer, along with other secondary troops around her.

I just reread Antonia's skill. It has a 25% buff "when leading." Is it the same case with Spanner?
