You see, the joke here is that when the man asks for ice cream, due to ice cream being not great for humans, the conventional size of ice cream consumption for many is low. The other man, in compliance with this, says "only a spoonful". However, the humorous aspect of this is that ice cream, while being bad for humans can also satisfy one's taste buds, so the original man brings in a comically large spoon so he can have more than conventionally expected while still staying true to what the other man said. Another aspect of this is that the original man is completely ignoring the fact that ice cream is bad for humans, thus causing the joke to be humorous.
