I think honestly, There is likely truth to the fact Muzan had empathy, Despite it being hard for many to realize. LIKELY Muzan buried such empathy deep down. BUT it is shown if you pay attention carefully in the smaller details. From his favorite lower moon, Rui. His reaction to Ubuyashiki's children. And among other things. You can tell while Muzan is quite evil, He is capable of understanding these positive emotions, he just buries it very deep down

I don't think he actually cared about his family he just knew how to act normal. Look at the family he had when he was a kid. His maid thought he was a sweet young master until he was so fed up with the demon slayers he broke and showed his true colors. He can pretend like the best of us but eventually he just moves on and doesn't care.
