That actually sounds like a really cool mod idea.

I remember a mod from back in the day that added water, oxygen, and sanity bars on top of the hunger bar (and all three would rapidly deplete in the Nether).

When the sanity bar got too low, one of the things that could happen is hearing creeper hisses right behind you.

Wish I could remember what the mod was called.

I actually have a game idea very similar to this. Its multiplayer, but basically all the players are schizo, and you have hallucinations of monsters, fake treasure/traps, and that your friends are trying to indirectly screw you over, or outright try to kill you. Meanwhile there's incentives to players for both sticking together, and betraying your friends, so the hallucinations actually have weight to them. Did your friend actually drop that grenade next to you, or was that part of the game's tricks?
There's a PvE focused "fight the hallucinations" route, and a PvP focused "obey The Old Ones" route
