I like the who-what-where-when-why and how mental jog - Nice!
I always start out with WHY and write down all the reasons for any project - I even prioritize them to remember what's most important because I know there will be trade-offs due to the Who and How.
I also use a spreadsheet to estimate costs and keep track of research (hyperlinks to products/plants), which is so helpful when a particular item is not available: I have all the key info about it written down so I can figure out a decent substitute.
And then I draw pictures. Pictures are so important for keeping everyone on the same page - they'll look at pictures, but not read a document.

This is so amazingly helpful, Coach. No project is really "hands-off"... I have learned the hard way that the licensed contractor that you hire to do the job may or may not be the actual person to do the job. He or she may be a hired hand who has very little skin in the game and bears very close watching. Even the actual contractor him or her himself may prefer to do things his way, and not yours. Buyer beware, and I have six P's to go along with your five W's... Poor Planning Produces Piss-Poor Performance.
