Actually I think a test-driver was badly injured or killed, when Cooper fitted an 2nd engine into the rear of a Mini Cooper sometime in the 1960s, because a weld securing one of the steering tie-rods to the sub-frame broke away during testing!
The dual-engine Mini Cooper was trialled after seeing it done in the Citroën, but scrapped after the prototype crashed!
