Disc 11 portrays the player running from an Enderman, who he kills at the end, hence the sound. The one recording is the Enderman and that’s why the disc is broken - it broke when he died.

Here is what I believe: music discs 5, 11, and 13 are played by their number (so music 11 would play in 11 seconds). Someone (could be Steve) went to find minerals but when mining he came across a ancient city and you can hear loud chain footsteps which could be the warden, the steel from a flint and steel can be heard, then fire which means 1. He was distracting the warden or 2. He was making a campfire. Once he didn’t hear the warden he started to walk until he heard a enderman of some sort so he started running until he realized he triggered the warden so then he starting building up or jumping while placing blocks. Once he made it to a structure of the ancient, he used a flint and steel to light up the portal and when he did he made it to a different dimension. There you can hear some lava, skulk, bones, skulk catalysts, and dirt/gravel. Until he/she heard a weird sound. It sound like souls, then it got quiet until she or he died but right before you hear loud chains, steel, souls, and screaming. The end

Disc 11 is probably connected to the Warden Lore as you see a player is running away from a creature and since he was caught the creature Turned him into what we know the warden, the reason why the disc is broken is the player dropped the disk and the creature stepped on it a few years later a miner took the disk fixed and the miner forget about it I’m sorry for the yap btw

The person in the black disc was being chased by a creeper it continues with disk 13 he falls into the water and the music just cracks the second music disc I believe it does contain the portal being lit but I also think the walking portion is an army marching. It’s a battle. Most likely against the wither as the wither was a threat to the ancient builders it’s not the same wither as we can make but instead, it’s one that’s way stronger I think of the wither storm but worse
