I worked at an old folks home. Dying on the shitter is extremely common. Both due to people straining to shit while constipated, AND when you’re having a heart attack it’s not unusual to feel the need to shit.

My grandmother almost died while she was on the toilet. She had myelodysplastic syndrome, and she passed out, she would have died there but my grandfather called an ambulance and they broke the door down.

She did live for a couple more years, but the disease got her in the end. Miss you Nanna and Pop.

There's Japanese folklore of Spirits who roam in the bathrooms of random or old abandoned buildings. An infamous example include a mythical yōkai or ghost that does a Matrix type thing for his (mostly female) victims; where he gives them blue or red toilet paper. But it depends on the outcome. If it's blue, they get SKINNED ALIVE, but if red, they get stabbed!
