Η εκκίνηση για τον Αυθεντικό Μαραθώνιο Δρόμο δίνεται από τον ειδικά διαμορφωμένο χώρο της Αφετηρίας, στην πόλη του Μαραθώνα στην Αττική.
Τα πρώτα 200 μέτρα είναι κατηφορικά και οδηγούν στην έξοδο από την εγκατάσταση της Αφετηρίας. Τα πρώτα 4 χιλιόμετρα παρουσιάζουν μια ελαφρά κατηφορική κλίση. Οι δρομείς τρέχουν επί της Λεωφόρου Μαραθώνος μέχρι το 4ο χλμ, οπότε και εισέρχονται αριστερά στην οδό Μαραθωνομάχων και την περιοχή του Τύμβου για τα επόμενα 2,2 χλμ. Η διαδρομή ακολουθεί σχεδόν κυκλική πορεία γύρω από το Μνημείο Πεσόντων (Τύμβος) στη Μάχη του Μαραθώνα.
Δίπλα στον χώρο της αφετηρίας βρίσκονται διάφορες αθλητικές εγκαταστάσεις που εξυπηρετούν τους κατοίκους της περιοχής.

The authentic Marathon Race starts from the Marathon Start Venue in the City of Marathon, located in the east of Attica in Greece.
The first 200m of the course are slopping, leading the runners away from the Start area. The first 4 kilometers go downhill. The runners run along the Marathonos Avenue up to the 4thkm, where they turn left and enter the Marathonomachon Str. (in memory of Marathon Battle fighters), and area of the Marathon Tomb (War Memorial), which they follow for the next 2.2km. The course follows a rather circular route around the War Memorial.
Adjacent to the starting area are various sports facilities serving the local residents.
