When you should use a TQ or Wound Pack??

I get the question all the time about when someone should use a tourniquet or wound pack a wound. The simple answer is that you put a tourniquet on major bleeding in the arm or the leg. We normally would pack hips and shoulders that have major bleeding that you can't get a TQ up high enough. Also, if you are a LONG TIME from getting to a trauma center you may want to consider would packing the injury and down grading the tourniquet. The tourniquets used in this video is the North American Rescue C-A-T TQ and the new SOFT-T Wide by Tac Med Solutions. (Both available at the Skinny Medic Store)

Great videos as always. As 2 suggestions to include later or for other viewers, when doing tourniquets from experience, 1- it's applied from between the legs, meaning you sit between the person's legs and then apply the knee to the inguinal area from the inside rather than out, which will let you get deeper and more effectively occlude the femoral, and 2- with male patients, to a "credit card" to the groin before applying the tourniquet. Make a credit card shape, slide up the legs, then get between the scrotum and leg, and push the testicles to the side and out of the way. It's something that wasn't taught and was overlooked, and we began implementing in my unit after a bunch of times where someone's scrotum would be sticking to the leg and the tourniquet was applied over it, and it prevents the hasty and panicked portion of applying and accidently getting either the scrotum or a testicle under the tourniquet.

I'm no expert, but i was told to place the tourniquet 2 to 3" above the wound site.Then if you need to you can apply a second tourniquet above the first tourniquet.

Have you seen or used the Recon Medical Gen 3 CAT tourniquets? They have an aluminum windlass and Kevlar reinforcement I believe. It's $16.97 on Amazon and appears to be really well made with a 5 star rating and 787 reviews.

You told the tale of a helicopter crewmember who was 10 hrs away from Medical help...tighten it, release it. Don't recall you mentioning wound packing or pressure dressing. I do recall you mentioning the injury in Black Hawk down would be a survivable injury.
Prepmedic disagrees about releasing a TQ.

So this is no problem for utube but Patron is blocking it? What's up with that? I wondered if it was an actual video of a major wound which could bother a sensitive person ... maybe. It is not. Nor do you discuss firearm or knife wounds.
I guess "they" don't have to explain ... but I sure would like to hear WHY.

Not suitable? That's crazy! On another note, I have a request or perhaps maybe an idea for your next video, if your interested. I've been trying to come up with a cargo pocket sized ALoK sack bleeding control kit. Now, what I'm thinking is something that would be ideal for someone with very minimal medical training, and also not include any hemostatic agent. I'm thinking this way because my wife and I have two 5 year old twin boys. She's needs something she could carry in the event one of them feel or got hurt where there was a some what major injury. I like the idea of the 5x9 and 4x4 and cling but I would really like to here your thoughts on it.

I THINK you have overstated the idea that the tourniquet should be put "as high as you can get it." Am I correct that what you meant to say is put the tourniquet 2-3 inches above the bleeding injury, UP TO the point that the tourniquet is as high as you can get it? Thanks for the clarification.
