I just finished Part 2 and almost everything is working GREAT! Thank you much for this tutorial! Very well done! The two issues I'm still having is 1) my Reset button doesn't reset the form and 2) all the data is still present after submitting the form. I get the 'Thank you' message but all the fields are still populated where your example shows the 'Thank you' message appear but the form is cleared?

Do not use nested tables, first table could be replaced with div and proper CSS. Less code needed.
