Exact thing used to happen to me on mushrooms. Periodically I'd get a black out and my body would collapse, once I smacked my face on my desk that I was sitting at and ended up watching myself in third person scramble to get up like 30 times like a record was skipping. Another time I was standing and fell like a bag of bricks, hit just next to my temple on the corner of a dresser with pretty sharp corners. That one convinced me that I had certainly died.
