The meaning of "90% effective" : if half of a population is given the vaccine and the other half a placebo, and 100 people out of the population are later hospitalized with COVID symptoms, and 90 of those hospitalized are from the placebo group, the vaccine is 90% effective.

Vaccines reduce symptoms. They do not prevent infection or asymptomatic spreading.

Drop in effectiveness :
Science magazine reports a significant loss in vaccine effectiveness over time. From March to September 2021 :

Moderna : 89% to 58%.
Pfizer : 87% to 45%.
J&J : 86% to 13%.
