Not sure what the song piece by piece was written about but it can be any parent that takes on the actual role of a parent versus the biological person that created the child. I can see a lot of stepfathers at a doctor children in this song. Also different generations maybe one generation didn't have as good of a relationship with their father so they make a better relationship with their child
