This actually allows for insanely fast transportation, just get “high” up so you can fall as long as you want, then have a pearl to pearl down whenever you want. Easy

You should do this with 3 other players, who each have their own orientation. P1: normal, P2: East, P3: West, and P4 is upside down

I feel like the problem is that you're trying to remind yourself what "down" is to the world and the mobs.
If you just keep focusing on what down is to you, there shouldn't be much of a problem. Just pretend the mobs are on the wall, instead of thinking of yourself as positioned sideways.
