Writing an Equation of a Line (that passes through two points) - Algebra

This video teaches students how to write the equation of line when given two points on the line. In particular, this video will show students how to find the slope between two points, then use the point-slope form equation to write an equation of a line in y = mx + b form.

In the book Calculus by Morris Kline, they show the slope intercept equation.  Then they say that if you are given two points on a line you can derive the two point form of an equation.  But I don't understand how they went from the slope intercept equation to the two point form of the equation.  I'm missing a step somewhere.
