Why was the pulse taken for 2 minutes and not 1 minute? or was the second minute for respiration?

do you need to do hand hygiene in between touching the patient for the assessment and documenting? It doesn't seem like this is the case on this assessment.

If news score 0 do we still have to escalate or not?

I believe a score of 3 you inform the registered nurse. As it says on their NEWS score parameters sheet 1-4 ..you need to inform. So you escalate.
