This patient presented with their cement retained crown loose. These can be tricky to figure out b/c you need to figure out not only where the screw channel is, but what type of driver you need. As well, you have to make sure the implant is still functioning well before you reuse etiher implant, crown or both.

Gotta love doing endo on implants! I've definitely "endo treated" loose cement-retained implant crowns and turned them into screw-retained. I always tell the patient that I made the crown more serviceable now if it ever gets loose again. They think it's cool lol. It's crazy how many cement-retained crowns are out there that could have easily been screw-retained!

Nice job....did you happen to have a Branemark screw lying around of the exact size you needed?
Just some observations....I would have removed the rest of the metal where you were making your "endo" type access; that amalgam placed there is going to be too thin and risk for fracture. Also, I would have definitely adjusted the opposing occlusion if possible, rather than thin out that porcelain so much that you are seeing the opaquer . And loosening abutment screws are typically a result of bruxism, I'd check into that too.

I bet that material on the inside is Plumbers Putty, I knew a dentist that used it for screw access holes

It's the abutment that's loose. Not the crown per se. The failure is at the abutment-fixture (screw-retained) junction, not the crown-abutment (cement retained) junction. Title is a little misleading.
And I've always wondered why people do not design an occlusal access hole on the crown that leads to the screw. Takes away all the guess work of drilling blindly.

Your access attempt is way off to the median of the actual access. The PA is a good guide for your access. Then the only variable is the bucco-lingual location of the access.

2022 Amalgam? Why? When i get caught spilling amalgam debris in the sewage my practice will be closed, the sewerage has to be renewed and i get a big fine. Thats why here every dentis has to use an amalgam separator and labeled as toxic waste. Specialised garbage disposal have to pick up the toxic waste. let´s think a moment why? Since 1999 for example Sweden has banned amalgam fillings due to the health risks of amalgam fillings. 50% mercury in every filling. I don´t get it why such a toxic substance should be put in a body today. In combination with other metals in a mouth has an even worse effect to our imunesytem. I treat a lot of cancer patients before radiation treatment, sometimes i have no doubt that the combination of bad implants with a mixture of several different metals has a severe role in the patients cancer developement.
