How come some surgeons do Lipo + Gland, and some only gland removal?

I’ve met 2 different surgeons here in Sweden. One of them suggests WAL + Gland. And with lipo he doesn’t do it under areola. While the other surgeon suggests only gland. What is your answer to that?
