I don't think schizophrenia or bipolar is as clear-cut as everybody thinks. Suffering from a lifetime of narcissistic abuse can look exactly like schizophrenia and psychotic illness. But when you get the people away from their abusers all the symptomology changes.

Schizophrenia can seem like Major Depression in the beginning, the prodrome, before psychosis. Schizoid PD can also seem like Depression, Autism or Schizophrenia. Mania with the psychosis and disorganized behavior can seem like Schziophrenia. Drug abuse can seem like many things. People may have multiple disorders, or develop the later on.

I guess it is a matter of asking many questions over time and being patient.

Which diagnosis could be fatally wrong if the psychiatrist does not discuss his interpretations with the patient. The main problem is that many of them generalise from his/her "in vitro" observation, not taking into account the possibility that a person behaves completely differently when not observed.
