How to DETERMINE Fetal Positions in Breech Presentation

Breech Presentation is a Malpresentation in which the fetus presents with the fetal breech [Buttocks] at the pelvic brim instead of the normal vertex in a cephalic presentation. With breech presentation the position determinant or the denominator now becomes the sacrum meaning anywhere it is pointing will be the position. The engaging diameter will be the Bitronchanteric diameter.
A breech presentation will still obey the principles of labour with changes in the movements made due to a change in the presenting part which in this case is the fetal buttock.
This video simplifies how to determine the Fetal positions in a Breech Presentation using the denominator which is the Sacrum.

Take note that most babies that would have been in breech would still undergo spontaneous version and present as cephalic in later pregnancy, those that do not may have to be assisted through External Cephalic Version [ECV]
