Hi.. I got approved for ltd my question is that still valid even if I lost I job cuz my flma is gonna expires soon?

The employer decides if you can file a claim? How soon can you even file for LTD if it does not kick in until 6 months? I have over 4 months of sick days but my employer automatically rejects leave of absence of more than 15 days. Don't you need medical history to file that claim and won't filing without good history hurt chances.. seems like going back to work puts you at risk for being terminated. Can filed claims be amended or revised along the way after being rejected?
