Very thoughtful video! I’ve become a strong medial triangle excision proponent myself— it adds a bit more work but I agree it closes much more nicely and in patients that need a larger bleph and so have the wider angle you’re discussing I believe they benefit from some of the more medial tissue excision the medial triangle provides but the medial triangle allows the tissue removal in a safe manner instead of carrying the regular bleph flap medial to the punctum and risking webbing.
