What is the point of choosing the A or D in the last question when they are identical?

Hi Sir, I have slight confusion regarding the last permutation combination question. I don't understand why we are doing 2c1 when choosing an A and D. Both the As and Ds are identical and it makes no sense to choose from them right? There has been sums like this in various other past papers and we never had to to choose between identical letters even if they repeated in the word. You can check paper 9709_52_Nov22 question 7c. I would be glad if you can clarify any conceptual error because I have exam in two days.
