Niki Lauda insisted Boeing redesign the planes engine reverse system. Without him Boeing wasn't going to bother.Unbelievable. He saved many lives because he wouldn't back down and made Boeing take the right actions to fix it. RIP Sir. What a man.

I just went to the crash site this morning (15/02/2019) Many pieces of the aircraft still there, such as rudder, cargo door, Flaps, window frame and one thing that made me feel sad is a burned paper that were painted by a kid.

I read that as the plane was plunging, it possibly broke the sound barrier, and the fuselage was not designed to cope with such stress. Very sad crash!

This happened because the FAA allowed Boeing to design the test to check for reverse thrust in flight and Boeing then designed the test for a flight which did not represent the actual flight conditions their planes would nornally be operated at.

The Boeing design was faulty, yet the FAA allowed it to be fitted on commercial aircraft.

Boeing and the FAA then went on to repeat this type of fiasco on the 737 Max, ensuring that hundreds more innocent people died horrifically and needlessly, but in the case of these crashes, Boeing blamed the Pilots, all the while knowing full well what caused the crashes and remaining silent.

After countless deaths no Boeing or FAA personnel were arrested or charged for the deaths they caused.

I hope Covid-19 bankrupts Boeing!

The flight data recorder was completely destroyed, so only the cockpit voice recorder was of use. Pradit Hoprasatsuk, the head of the Air Safety Division of the Thailand Department of Aviation, stated, "the attempt to determine why the reverser came on was hampered by the loss of the flight data recorder, which was destroyed in the crash".[15] Upon hearing of the crash, Niki Lauda traveled to Thailand. He examined the wreckage and estimated that the largest fragment was about five metres (16 ft) by two metres (6.6 ft), which was about half the size of the largest piece in the Lockerbie crash.[16] In Thailand, Lauda attended a funeral for 23 unidentified passengers, and then traveled to Seattle to meet with Boeing representatives.

The official investigation, led by Thailand's Aircraft Accident investigation Committee, took about eight months, and was released with the "probable cause" stating: "The Accident Investigation Committee of the Government of Thailand determines the probable cause of this accident to be [an] uncommanded in-flight deployment of the left engine thrust reverser, which resulted in loss of flight path control. The specific cause of the thrust reverser deployment has not been positively identified."[17] Different possibilities were investigated, including a short circuit in the system. Due in part to the destruction of much of the wiring, no definitive reason for the activation of the thrust reverser could be found.[5]

As evidence started to point towards the thrust reversers as the cause of the accident, Lauda made simulator flights at Gatwick Airport which appeared to show that deployment of a thrust reverser was a survivable incident. Lauda said that the thrust reverser could not be the sole cause of the crash.[18] However the accident report states that the "flightcrew training simulators yielded erroneous results"[5]:21 and stated that recovery from the loss of lift from the reverser deployment "was uncontrollable for an unexpecting flight crew".[5]:41

They really don't know what brought down this plane.. thrust reverser deployment in flight is really not as big a deal as you'd think. The FAA requires all aircraft equipped with thrust reversers to pass a flight test in which the reverser is deployed in normal flight. The Boeing 767 passed this test with no issues. All that happened was a little control column shake and a slight yaw to the side of the reversed engine. The pilots had no problem whatsoever controlling the aircraft. Under no circumstances should thrust reversal cause total loss of control leading to an in-flight break up of the aircraft.
