If i remember correctly Enzo's vote from bb12 was accidently revealed because he put his key in shot of the camera just barely. But instead of putting the camera on Julie for the final vote they pointed at the contestants to get their reaction instead.

I might be misremembering though.

I've said it before and I'll say it again. The biggest production error was in BB20 when production allowed Tyler to win the final 8 HOH instead of Haleigh when he clearly broke the rules

When I think of production screw ups, I think of the first HoH competition on BB21. Cody was able to see the pegs move from the prior player and everyone else had to wait for the pegs to stop moving. It looked like an unexpected oversight but definitely locked Cody as the first HoH.

Looking at how the light reflects off of the tubes the golf ball falls into - those aren't graphics, there are actual cameras on the tubes in the back (those tubes likely used for a different comp related to filling them up). Either way, that was definitely a screw up.
