Given the mention of "walls that can whisper", the building itself seeming to phase in and out of existence, and the voice being audible yet reverbed inside the dark resturaunt, I'd wager to guess that the entity IS the Mcdanooj's building itself, with it communicating through a psychic link. The associated entities are either its' servants/"employees", extensions of itself, or extra-dimensional parasites that have clung to it(considering how it *warns* you about "the entity")
