==FARM ISSUE SOLVED==
Your build works perfectly fine in Singleplayer and new "Paper" servers. The issue is due to a bug in an outdated server plugin.

-- The"easy" solution --
Ask the staff to update the following server plugin:
"Multiverse-NetherPortals"

--- The "hard" solution ---
Move the portal and associated contraption in the nether.
Your coordinate when standing on the two portal blocks should be:
X: 199 / 200
Y: 117
Z: -12
The hopper clock can be placed on either side of the gate.

UPDATE: issue has been replicated by おかゆにぎり and has been identified as a misbehaving plugin. Multiverse-netherportals require an update. The portals do NOT need to be moved. All information below is irrelevant now but will be kept here. I should note that I didn’t test this myself but I need to sleep so I’m trusting this guy.

i recreated the entire farm using the EN seed and these are the portals that work (frame inclusive). when using f3 look at the "targeted block" coords on the right hand side. y coords are bottom of portal

remember find "targeted block" on f3 and y coords listed at bottom of portal and it includes the frame

Basically, the portals just send you or the mobs to the closest active portal, always, that's all it's doing. There's no linking, order doesn't matter, just generally the closest portal is the one you came in from because people don't build portals too close.
In this case, the nether side portal has 4 overworld portals to choose from that are all close by, and the closest one is one of the lower ones instead of the upper one as it should be.
All the portals in the overworld select that one portal in the nether chamber that's closest, that's all working, now you need that portal to send the guardians to the upper overworld portal.

So, you have to move the nether side portal further away from the lower portals and closer to the upper portal, even if it isn't the exact coordinates. 197 on the x axis like the (possible) messiah said might work, though a few blocks further wouldn't hurt. As long as it's within 250 blocks in the overworld it'll still connect so the further away from the lower portals the better.

What a journey this stream was. It will definitely be worth it in the end once it finally works, but hopefully the process from now on will be simpler thanks to one guy. That's not me though. Nature-sempai I solved it! Just replicated your portals in minecraft precisely in the EN seed. The problem was amost likelly an orientation problem. The portal works perfectly in an east-west configuration, when the two obsidians in the bottom center of the portal IN THE NETHER are at "x 198 y 117 z -22 and x 199 y 117 z -22 each! I can record a video later if necessary. Btw tanks for the ginormous stream, you always make my day!! If you guys could like it so she can see, it would help :D

I believe it had to be x=200 or greater. The bottom portal is x=1553 and the top is x=1600 according to chat, and in that case, when a guardian goes through the portal in the nether that’s at x=199 or smaller, it’s gonna be sent to the bottom, since 199×8=1576 and it’s one block closer to the bottom than to the top. If the portal in the nether is at x=200 or greater, it becomes closer to the top portal.

We may not have finished the project today, but I think you made some good progress, it was a wild ride! I don't have the minecraft experience to be THE GUY, but the general idea is that the bottom three portals need to send to the nether chamber and then the nether chamber portal sends to the kill chamber.

Since portals aren't one-to-one linked and choose a target based on distance, we need to fiddle with the placements of the portals to figure out where they should go. It's easiest to move the nether chamber portal, so start by figuring out its position by taking (floor(x/8), y, floor(z/8)) of the coordinates of the killing chamber in the sky, so we can establish that the nether chamber portal will send to that one. Then, we need to check to positions of the portals of the siphon portals on the water's surface to make sure that the nether portal doesn't output to them by accident. Hope some of these ideas help, I'm fairly sure I've grokked how it's supposed to work, but I don't have the numbers on hand to crunch it now. We might be getting got by the algorithms nearest selection component, because I think that can take the Y value into account... but again, I 'm no minecraft expert and i'm mainly reasoning by the logic as described on the wiki.
