What is the difference between Monitoring and Evaluation?

While monitoring and evaluation forms an important part of any project and program, there are some important differences. Monitoring is regular and carried out to correct the process so it happens at activity and output level, where as Evaluation usually happens at the end to assess the impact of project or program. So evaluation happens at outcome or impact level and informs policy decision.
