Well she could always become a “doctor” herself. Maybe that is one way to solve the endless child issue. Kind of like the way Buffy ends; from the chosen “one” to the chosen “many”.

I really hope Donna doesn't die. I don't want her arc to go out with a whimper, but I just don't want her permanently gone either. I'm not even sure what a satisfactory ending this time around for Donna would be. Has it been confirmed Rose will be travelling with Ncuti's 15th Doctor? Maybe her role is just a cameo as we'll be getting Ruby as a companion. I am really looking forward to the chemistry between Donna and the Doctor. I imagine it will be a bit different with 14 being a new (kinda) iteration, but Catherine and David really worked well together.

RTD has never killed a companion. And I don’t think he would. He likes to make us THINK he’s going to but then he does something else (strands them in a parallel universe, wipe their memory)

Rose Noble is going to be in series 14 but she isn't traveling with him. The Doctors new companion is played by Millie Gibson

I think Donna dying will make Rose not want to travel with The Doctor, and that's why Ncuti's companion will be Millie Gibson's character and not Rose.

Donna will die, the Doctor will die and become the next, her daughter will be the one in danger, and she will want to know the history of the Doctor, Donna, and how she came to become the most important woman in the universe. And that's why she will travel with the next Doctor.
