In this video we will be examining three incidents in which poor design choices led to fatal accidents. I'm going to talk about a badly designed UI in a medical software, that led to a death of a little girl, the terrifying radiation machine called Therac-25 and the Air Inter Flight 148, where bad display design resulted in a plane crash that took 87 lives.
