Passive voice - Mistakes were made. A mistake was made. 

Active voice - I (he/she/they/we) made mistakes. I (he/she/they/we) made a mistake. 

Notice how the passive voice structure focuses on the action (mistake(s)) rather than the people responsible for the action. This type of sentence structure can certainly be used when you don’t know who performed the action - but it can also be used when you don’t want to focus on who performed the action.
